5 Things Korean Dermatologists Do First That We Do Last

Korean women are not using better ingredients than you.

Most of what is on a Seoul bathroom shelf is on yours too. Vitamin C, niacinamide, centella, retinol, SPF. Same molecules, often the same suppliers.

What is different is the order they go on in, and how early the first step starts. In Korea the sequence is taught as clinic procedure rather than personal preference, and it is close to the reverse of what most of us picked up from the internet.

Here are five things Korean dermatologists do first that we tend to do last, or not at all.

The order is the product.

  • ×We open with an active and only repair when something already hurts.
  • × We aim products at a line or a spot, not at the layer making them.
  • × We buy on claims because almost nobody prints the concentration.
  • × We build routines we choose in January and abandon by March.
  • × And we treat repair as what you do after damage instead of before it.

They repair before they resurface

#1

They repair before they resurface

(we do it the other way round)

Our routines open with an active. Acid, retinol, vitamin C, something that works on the surface, and repair is what you reach for afterwards if your skin complains.

Seoul treats repair as step one whether or not anything hurts. The barrier goes first, every night, and the actives sit on top of a layer that has already been rebuilt.

The reason is not gentleness. It is that an active applied to a depleted barrier spends most of its effect being absorbed badly and tolerated worse.

See what the first step actually is

#2

They treat the layer, not the line

(the line is the symptom)

Western skincare is organised around visible problems. A line, a spot, a patch of pigment, each with a product aimed at it.

Korean clinics work one level down, at the layer where skin renews itself, on the argument that lines and slackness are what a slowing renewal layer looks like from the outside.

The ingredient they use for it is PDRN. It appears on an ingredient list as sodium DNA, and it came out of wound care rather than cosmetics, which is why it is applied the moment a laser treatment finishes.

A moisturizer cannot get there. That layer sits under the one a cream is formulated to sit on, which is roughly the thickness of a sheet of kitchen foil.

What the two steps cost here

What the clinic version of the same step costs

  • PDRN skin booster, one session$300 to $600
  • A course of three to four sessions$900 to $2,400
  • Collagen stimulating injectable, per vial$600 to $1,200
  • Consultation, usually chargeable$50 to $150
  • Maintenance, twice a year after that$600 to $1,200

A typical first year$1,500 to $3,600

The two creams, six months$74.99

Typical US and Korean clinic ranges for the injected version of this repair step. Ours is the topical version at 20,000ppm, at the Buy 2 Get 1 Free price, and it is not a subscription.

They count concentration, not claims

#3

They count concentration, not claims

(a label can say centella at almost any dose)

Centella asiatica is on hundreds of Western labels. What is rarely on the label is how much, because a trace amount still earns the ingredient a line on the box.

The Korean convention is to state the number. The firming step here carries 30,000ppm of TECA, which is the concentrated form of centella, and that figure is on the packaging because in that market it is the thing people compare.

Both creams are made by Centellian24, owned by Dongkook Pharmaceutical, which has worked on centella extraction since 1968 and holds four patents on the extraction alone. The cream has sold over 100 million tubes in Korea, a country of 52 million people.

  • 30,000ppm TECA, stated on the pack
  • Four patents on the extraction itself
  • A pharmaceutical company, not a beauty brand

They pick the routine you will still be doing in March

#4

They pick the routine you will still be doing in March

(sixty seconds, not forty minutes)

The ten step routine is the thing Korea is famous for abroad and the thing it has largely moved on from at home. What replaced it is short and repeatable.

Cleanse. PDRN first, pea sized, patted in. The firming cream straight over the top. Go to bed.

Nothing to rinse, nothing to plan an evening around, and nothing that stops working if you skip a Tuesday.

This is not a convenience argument. Almost nothing fails on the formula. It fails at the point it becomes a chore, and a routine you abandon in week two does nothing at all regardless of what is in it.

See the two step routine

Sixty seconds, every night

  1. Step 1Cleanse and leave the skin slightly damp
  2. Step 2PDRN repair cream first, a pea sized amount, face and neck
  3. Step 3Time Reverse over the top, same amount
  4. MorningSunscreen. Nothing else changes

    What is actually in them

  • PDRN at 20,000ppm, the repair step Korean clinics inject
  • 30,000ppm TECA, the concentrated Centella fraction
  • Madecassoside, the wound healing compound in both creams
  • Niacinamide for tone and pores
  • Four patents on the extraction, held since 1968

What to honestly expect

  1. Week 1 to 2Skin feels calmer and less tight. Nothing you could photograph yet
  2. Week 3 to 5Redness settles and texture smooths. Most people stop here and should not
  3. Week 6 to 8Firmness is the slow one. This is where the jawline shows up
  4. Week 8+It compounds. This is the point of doing it nightly rather than sometimes

Who this is actually for

    This is for you if

  • You already own good products and want the order right
  • You have been layering actives onto skin that never got repaired
  • You want the concentration printed on the box, not implied
  • You will do two steps at night and nothing more
  • You would rather test the order for $74.99 than book a course
The Madeca Best Duo

Direct from Korea rather than through a distributor

The two creams together are $39.99. Retail for the pair is $66.50, and the gap is there because we import direct rather than through a US distributor.

Two 50ml tubes used once a night is roughly two months. That works out at under seventy cents a day.

Most people take the Buy 2 Get 1 Free, which is six months of both creams for $74.99. That is forty two cents a day, and it is the one that stops you running out halfway through the stretch where the change actually shows.

  • 4.8 out of 5 across 748 reviews, the critical ones still up
  • 30,000ppm TECA, not a token amount added for the label

Can I just buy one?

You can, and plenty of people start there. But the firming cream on its own plateaus, because the slackness is coming from a layer it cannot reach. And the repair cream on its own does not touch the lines on the surface.

One gets you half. Both gets you the order that Seoul actually uses.

Where does this sit in my current routine?

At night, after cleansing, before anything else. If you keep a separate serum, it goes on after these two rather than before. Sunscreen in the morning is unchanged.

How fast will I see anything?

Calmer skin in the first week or two. Texture around week three to five. Firmness is the slow one and it lands closer to week six to eight. Anyone promising a jawline in seven days is selling you another lap.

Do I have to do it every night?

Nightly is what the reviews are based on. Five nights a week still moves. Twice a week does not, and I would rather tell you that now than take your money.

Is PDRN safe, and what actually is it?

Polydeoxyribonucleotide, derived from salmon DNA. Korean dermatology clinics have used it for years and it is what they inject in the repair step. This is the topical version at 20,000ppm from Dongkook, a pharmaceutical company established in 1968. It is a cosmetic, not a drug, and it is not intended to treat any condition.

Will it break me out?

The firming cream is rich, which is why it is a night cream. Patch behind the ear for two nights if you are reactive. Both passed skin irritation testing. Skin is skin and mine is not yours.

What if it does nothing for me?

Thirty days, money back, and you are not asked to post the creams back. That is the only reason I tried it instead of keeping the appointment.

#5

They let you send it back. Thirty days, and you keep the creams.

(no form, no return label)

This is the part most pages bury and it should not be buried.

Use it for thirty days. If nothing has changed, email and you get your money back. You keep the creams. There is no form, no return label, and nobody asking you to explain yourself.

That is possible because the repeat purchase rate is what it is. 4.8 out of 5 across 748 reviews, and the critical ones are still on the page because deleting them would tell you less than leaving them up.

None of this needs you to throw out what you already own.

It needs two creams going on in the order a Seoul clinic would use, sixty seconds a night, and thirty days to see whether the order was the part you were missing.
